LaGrange:
8:30am – 11:00am and 1:00pm – 3:00pm Mondays and Wednesdays.

Shipshewana:
9:00am – 11:00am and 1:00pm – 2:45pm
On the first and third Thursday of every month. This clinic is located in the Wolfe Community Room in Shipshewana. Walk-ins are limited to the number of children we can work in around appointments.

Topeka:
9:00am – 11:15am and 1:00pm – 2:45pm
On the second and forth Thursday of every month. The clinic is located in the Topeka Fire Station. Walk-ins are limited to the number of children we can work in around appointments.

Walk-in Clinics operate on a first come first serve basis. They are limited to the number of children that can be served by the available nursing staff.

Appointments
We have appointments available at all three clinic locations. To schedule an appointment please call (260)499-4182. We are out of the office on Thursdays for off site clinics.

Cancellations
Clinic days and times are subject to change due to illness, weather, and staff in-services. If time allows, clinic changes will be reported on Thunder Country Radio 105.5. If school is cancelled due to weather, immunization clinics will be closed.

Clinic Requirements
To receive immunizations you must:
1. Be the child’s parent or have proof that you are the child’s legal guardian OR you are at least 18 years old and have a note from the child’s parent or legal guardian, giving you permission to bring the child. A note is needed for every visit.
2. Permission forms must be completed each time you visit the Clinic.
3. Child’s immunization history is required every visit.
4. Child must not have a croupy cough, fever, vomiting, or diarrhea in the past 24 hours.

TB tests are given on Monday and read on Wednesdays in LaGrange by appointment only.
